Information is collected only when you give it, through booking an appointment, filling out a form, calling, texting, or emailing. That includes:

Name, phone number, and email address
Appointment details and scheduling history
Payment information, processed by Square and never stored here
Information you choose to share about your health when you book or attend a visit

How information is used

To schedule, confirm, remind you about, and follow up on appointments
To respond when you call, text, or email
To take payment for a visit
To keep the records the practice is required to keep

Information is not sold. It is not rented. It is not traded.

Text messaging

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

If you give your mobile number when booking or when you contact the practice, it may be used to send appointment confirmations, reminders, and replies to messages you send.

Consent is not a condition of care or purchase.
Message frequency varies and is tied to your appointments.
Message and data rates may apply.
Reply STOP at any time to opt out. Reply HELP for help.
Carriers are not liable for delayed or undelivered messages.

Who information is shared with

Only with the services required to run the practice, and only as far as those services need it:

Square, for scheduling and payment processing
The email and messaging providers used to reach you
Anyone the law requires, or anyone you authorise in writing

Health information

Anything you share about your health is treated as confidential and is not disclosed without your written authorisation, except where the law requires it. Please do not send detailed health information by text or through a web form. Bring it to your visit or use the intake form provided to you.

Ask what information is held about you
Ask for it to be corrected or deleted, where records rules allow
Opt out of text messages by replying STOP
Opt out of email by replying and asking

Children

This website is not directed at children under 13 and information is not knowingly collected from them. A minor seen in the practice is scheduled and consented to by a parent or guardian.

Security

Reasonable measures are taken to protect the information held. No method of transmission over the internet is completely secure, and that cannot be guaranteed.

If this policy changes, the updated version will be posted here with a new date.
